You plus up to 4 family members. One of them can be another adult, and the rest are children under 18, so a plan with a partner on it has room for 3 children. Additional adults are $12 a month each, up to 2 more, on top of the 4. A spouse or domestic partner has no age limit; a child who ages onto the plan stays eligible through age 26.
Not when your team enrolls and pays Bivvy directly. That is a consumer purchase and it creates no benefit plan for your business. It does change if you pay for or contribute toward memberships: an employer that funds medical care for its employees is generally treated as sponsoring a group health plan under ERISA and the ACA, whether or not the product is insurance.
